Email me your postal address and I'll send you a disk with *FORM40 and
*FORM 80 on it.
 
(What disk do you want?  40track? 80 track? 5.25"? 3.5"?)
 
Alternatively, try http://www.mdfs.net/Software/HADFS/Source/Formatter and
see if you can transfer to the BBC via the serial cable.
