Subject: Disc fault 04

Did anyone ever find out what disc fault 04 is? Because I just got it from
the Master 128 DFS while trying to debug my own 1770 FDC code, and as per
usual, the docs start from Disc Fault 08...

It seems that somehow in the process of trying to implement this Opus DDOS
capability, I've accidentally knackered the existing code...
