Issue 3 does exist, I have one with a couple of unusual upgrades.  I've
also got an Issue 2 with Econet which is probably fairly rare:

The numbers started again for the B+, with a copyright date of 1984
although I think it only ever went to issue 1.

I'm almost certain that 5 & 6 never happened; I think Pete Turnbull 
knows the story there (as has told it to me in the past, and I've since 
forgotten, and I don't have any useful archives to hand ;)

I'm not certain I've ever seen an issue 3 either actually. 7 is by far 
the most common in my experience, with 4 next.

Hmm, not sure. The only B+ photo I have is the one in my ACW, so there's 
a bazillion cables in the way.
