Yes, I have, and I'm using it on a permanent basis.
I use a Pinnacle PCTV Rave card. Just connect composite
video.

Yes. Screen modes (even tweaked ones) need to display
correctly on TVs, so will be acceptable for a TV card
as well (15 kHz linerate, 50 Hz interlaced framerate).
