Could be it's not actually writing anything to the disc at all. Try 
formatting a disc on another machine, then try formatting it on the one 
that's giving you problems.

What happens if you *CAT the disc it's failed to format? I think the 
formatter writes a lower-case 'e' all over the disc, so you end up with a 
disc with a title of 'eeeeeeeeee') then it's writing to the disc.
