PFM, as colleague would have said.

It's complicated, and hard to explain without a good knowledge of
realtime problem solving. But the essence of it was a) recreating
the original output pixels (without over- or undersampling and/or
interpolation) and b) rendering them (while line doubling) so the
monitor doesn't freak out and show either horizontal or vertical
jitter.

I may write some more in-depth account once I have finished the
'mode 0 through 6' part, and have tested it thoroughly on all my
retro stuff.
