The Dissem/Monitor package from JGH-PD will do that.  Dissem is a rom
image providing various disassembly functions, and Monitor is a Basic
program that extends those features, providing, amongst other things,
disassembly from disk.
 
I also have Monitor80 and Monitor65 which will disassembly Z80 and 6502
respectively without the need for the Dissem rom support functions.
 
I can email them to you, Mon65 is about 8k.
