I'll neglect the "battery" versus "cell" usage which would belong in a
separate thread... ;-)

I don't recall a single-cell device that was 4.5V.  The original was a
3V AA-size lithium cell, and that was what caused the problem.
 Provision had been made for a charging circuit, but the cells used in
production were not the same as the ones used in development, and
couldn't withstand the charging.  I never came across a verifiable
example of one exploding (though it would have been theoretically
possible) but matters came to a head when a machine in a hospital got
very hot.
