If we're going to start this list, I can confirm the Gigabyte 686BX (Pentium 
2/3 slot 1), ECS K7S5A (Athlon socket A) and ECS nForce3-A939 (Athlon socket
939) all support FM correctly. If chipsets are useful, these are Intel 440BX,
SiS 735, and nForce 3 respectively

Another item of interest would be the drive used, as I've had Omniflop work
correctly with HD drives but never 40-track DD. I've got an 80-track DD drive,
but haven't tried it yet.
