I suspect it's largely a non-problem as pretty much no-one uses pulse
dialing phones now.
I'm not sure there are any pulse-dialling only exchanges left (and if
there are I doubt they would support ADSL) and pretty much all phones will
support DTMF - the only exceptions I can think of are rotary ones and some
very early push-button ones.
