After staggering to bed at 2am last night...

No longer need a way of storing the clock setting, code works it out
by frobbing the hardware.

Updated *MIDI command at http://mdfs.net/Software/Music/Utilities/
automatically works out what clock setting the 6850 ACIA needs
setting to (can be overridden), so now works correctly with the
EMR MIDI interface as well as the M2000, ETI and JGH MIDI interfaces.
Have also completely rewritten the documentation. I've also reformatted
the source code in my ongoing quest to get all my source code in a
readable format so as to make it available.
