Subject: IDE patches for BBC B/B+ ADFSes

I've just finished creating a reverse engineered patch to create the ADFS133 
ROM I have.

I also re-instated the option to retain the context on Ctrl-Break, and added 
the option to execute !BOOT from the CWD.

These options can be controlled by variables on line 125.
